Lifestyle Coordinator Position Full TIme [ annual gross $60,000 - $69,999]
Community Connection Inc. is a community-based organisation that provides personal, one-on-one lifestyle support and assistance to individuals with a disability and their families. The service operates in Townsville and offers choice and control to individuals living and participating in the community.
The Lifestyle Coordinator role will lead teams that support individuals with high and complex behavioural support needs. Our NDIS services are individual-driven and tailored to actively support every NDIS participant to achieve the goals outlined in their NDIS plans. This includes supported independent living, capacity building, and core supports.
ABOUT THE OPPORTUNITY
Reporting to the Strategic Manager, the Lifestyle Coordinator will be responsible for managing a caseload of participants who receive direct supports through Community Connection. This position is primarily responsible for providing staff supervision and support; rostering, communications with key stakeholders and the management of behaviour support and restrictive practices.
The Lifestyle Coordinator coordinates this service within a model that supports individual aspirations, growth, and change in a strengths-based approach. The position is also required to participate in the organisation's after-hours on-call roster - additional remuneration applies.
Salary is above award Level 5, [$60,000 - $69,999 gross income] and conditions are in accordance with the Social, Community, Home Care, and Disability Services Award. We offer a family-friendly work environment and a range of practice and professional development opportunities.
