Community Connection is a recognised provider for other funding support arrangements and programs such as “Your Life, Your Choice” – which enables Self-Managed arrangements.

Self-management means that the individual and / or their family take full responsibility for arranging and delivering services with minimal support from Community Connection. In this model the individual and/or their family are the employers and manage all employer responsibilities, and Community Connection is simply there to help with government linkages and administration.

There are fees applicable for this service.

Community Connection is always open to discussing and developing new ways to support individuals with disabilities and their families.
